Japanese adult video (known as AV or JAV in Japan) is censored with a mosaic due to Article 175 of the Japanese Cirminal Code, which prohibits the sale/distribution of obscene materials. Most mainstream JAV production companies belong to 'ethical associations' that determine what's acceptable to show (similar, in my opinion, to the American Motion Picture Production Code from the '30s to the '60s). Up until the 90s showing pubic hair at all was considered obscene which is why if you look at older JAV and the mosaic covers the entire lower torso regions of the participants -- I read at some point there was a large number of porn stars that faked sex during these days.
I think the mosaic goes to the heart of Japanese (and to some extent much of Asia's) belief systems where social harmony is paramount. People are willing to overlook immorality as long as there's a 'moral' fig leaf in place. In other words, Japanese people (or at least the government) will ignore the fact that a 19 year old girl did some pretty outrageous things on camera for money because the mosaic covers up the actual deed.
